I am aware of the Young Mayors program, understand the nature of the activities and authorise my child’s participation in these activities.

I consent to The Foundation for Young Australians (FYA) collecting my child’s personal information for the purpose of their involvement in the Young Mayors program.

I consent to FYA using any of my child’s feedback or data for research and/or evaluation purposes.

I give permission for my child to be photographed and filmed with their consent, and understand that my child’s image, words and story may be reproduced across FYA’s website and social media channels, and the participating Council’s website and social media channels

I understand that the duration of this consent is for the length of my child’s participation in the program, and I can revoke consent at any time by contacting FYA on the details below.
